Please make sure that you fill out all information on this form as it will be used to inform you about next steps.

*This is not a background screen application - Anyone who will have access to the sidelines this Fall will be required to complete a background screening. A link will be sent to you to prior to being assigned to a team.

This application does not guarantee your assignment as a Coach or Assistant. The board reviews the applications with the Director of Coaching to make decisions.
